Schedule An Appointment

All Medical Cannabis evaluations in Florida must be performed by a qualified physician.

Dr. Ryan is a qualified ordering physician under Florida Law. She is able to qualify medical cannabis patients and to make recommendations for low–THC (CBD) and full strength medical cannabis. All evaluations will be completed by Dr. Ryan personally.

Pre-Visit Requirements

  • Must be at least 18 years of age.
  • Must be a legal Florida resident, with valid proof of such residency. Examples include, driver’s license or state ID.
  • Must meet the criteria of a qualifying medical condition approved by the FL Office of Medical Marijuana Use/OMMU
  • Qualifying Conditions for Medical Cannabis Treatment Include:
    • AIDs
    • ALS (Lou Gehrig’s Disease)
    • Cancer
    • Crohn’s Disease
    • Chronic nonmalignant pain
    • Epilepsy
    • Glaucoma
    • HIV/AIDS
    • Multiple Sclerosis
    • Parkinson’s
    • PTSD
    • Any condition that is a similar type or class of the 11-conditions listed above (for example, “debilitating anxiety and PTSD” would be considered similar conditions)                                                                                                                  Examples of like conditions… Anxiety, Alzheimer’s, Anorexia, Arthritis, Asthma, Autism, Bipolar, Chronic Fatigue Syndrome, Chronic nausea, Chronic pain, Depression, Fibromyalgia, Chronic headaches, Huntington’s disease, ADD, Insomnia, IBS, Lupus, Migraines, OCD, Rheumatoid arthritis, Tourette’s syndrom, Ulcerative colitis.
  • Must have your medical records or documentation at least 24 hours prior to your doctor’s visit.  If you need help with your DIAGNOSIS, please reach out to us so we can help you.

In order to be scheduled for an appointment you must first complete the online paperwork using the link above. After we receive your paperwork and info about your  medical records, we will call you to schedule an appointment.

Please be sure to include your Doctor’s Phone and Fax Number so we can send for your medical records. If you do not provide this info, there can be a delay in getting your records in a timely fashion.

NOTICE ON THE COLLECTION, USE, OR RELEASE OF SOCIAL SECURITY NUMBERS

Florida law requires that public agencies provide individuals with a written statement identifying the state or federal law governing the collection, use, or release of social security numbers for each purpose for which the public agency collects an individual’s social security number. The collection of social security numbers by the Florida Department of Health is either specifically authorized by law or imperative for the performance of the Florida Department of Health’s duties and responsibilities as prescribed by law. This notice is provided pursuant to Subsection 119.071(5)(a), Florida Statutes. For the Medical Marijuana Use Registry Identification Card Qualified Patient Application, social security numbers are collected and used for identification purposes to ensure that the number identifier assigned to the qualified patient is unique and matches the identity of the qualified patient, as authorized by sections 119.071(5)(a)2. and 119.071(5)(a)6., Florida Statutes. Social security numbers collected for this purpose will remain confidential.
